Job Description
- Clinical Pharmacist is ensuring safe, appropriate and cost-effective use of medicine to improve patient safety.
- Clinical Pharmacist will plan to organize and monitor for delivery of clinical pharmacy services at DHQ Hospital Badin (Managed By: IHHN)
- Clinical Pharmacist will provide services to entire medical & surgical specialties & to participate with consultant rounds.
- Monitoring/Assessment of patients to collect specification, interview, patient to identify for proper therapy.
- The Aim to improve compliance & co-ordinate with multidisciplinary team in educating patients, attendants, doctors, nurses, and paramedical staff.
- To achieve medicine reconciliation processes on admission level to enhance patient safety.
- To Valuate specific pharmacotherapy by evaluating appropriateness of drug therapy, assessing effectiveness and identifying potential interactions or adverse effects.
- Clinical Pharmacist will be responsible to enter detail notes on admission in progress notes and routine notes when review patient medical profile.
- Design a comprehensive pharmacotherapy plan for patient specific problems and previously justifies recommendations based on patients specific pharmacologic, pharmacokinetics, pharmacodynamics, pharmacogenomics, Pharmacoeconomics, ethical, legal and evidence based information or standard practice guidelines.
- To provide drug information to health care professionals and document accordingly.
- To participate in the training of pharmacists, pharmacy technician, pharmacy assistants & Trainees
- To maintain the hospital formulary & review the non formulary requests.
- Monitoring of crash trollies, floor stock on monthly basis.
- To Conduct CMEs, CPE, Webinars
- Monthly reports of KPIs, Interventions, expiry, near expiry, slow moving & dead stock.
